Case Study: Multi-patient Charting

How can we decrease the amount of time it takes to document therapy sessions and improve accuracy?


Problem:

Group therapists must document some of the same information for all patients during their group therapy sessions. This documentation can take lots of time, and can lead to mistakes. How can we decrease the amount of time it takes to document therapy sessions and improve accuracy?

Our team consisted of a project manager, multiple designers, and 10-15 developers.

Process.

Our process included:

  • Interviews with therapists

  • Observations 

  • Wireframes

  • Multiple rounds of designs

  • Usability tests

Requirements.

  • Ability to set up groups of patients (these groups can be reused over the course of treatment)

  • Ability to fill out session details for all patients at one time

  • Have same form queued up for all patients in the group 

Solution.

Our solution consisted of three pieces:

  • Home page

    • Creation of a home page which allowed therapists to see their documentation for all their sessions in one place 

    • Acts as a launch point for creating new sessions

  • Session Details: When setting up a new session, a short form is launched. The information from that form will flow into each patient’s record.

  • Charting:

    • Patient list displayed on left

    • Each patient has same group session details–if modified, they cascade to other patients

    • Individual details/forms displayed below for specific documentation

Success Metrics

  • We were able to cut documentation time in half for therapists, allowing them to spend less time on their computers and more time with patients.
